This icon was made in Adobe CS4 on a Mac, but should be pretty easily translatable.
I began by rotating the entire image 15 degrees counter clockwise because I wanted him leaning forward rather than back, then cropped it, did a smart sharpen, and changed the image size to 100x100 for my base.

I then made a copy of that layer, just in case I wanted to screw around with it, so I'd have a spare hanging out.
For the next layer, I used one of Mustie's Brushes to create a mask, then did a gradient fill layer with a pale grey->white gradient.

Next I applied a gradient layer on blending mode screen. I can't remember the settings for the gradient layer, or where I got them (because I can't find it in my gradients!! this confuses me....) so I've included a picture. It has five stops on the colour bar, all shades of brown down to yellow.


Added a curves layer next to deepen the shadows because it was far too pale with the screen.


And the very last item was a brush made by Mutsie's Brushes, one of her text brushes (well, pseudo-text *smiles*), to give it some weight in that upper corner.

And that was it! Just a few small steps to this one.
