While I was doing the banner, I thought I might as well investigate the icon, so here's the tutorial. Icon was made in PS CS5 on a Mac, uses gradient and gradient maps.
I selected a largeish piece from the source picture and made a new 100x100 document, dropped my selection in as a new layer, and rotated it until I liked the look of the crop. (Let's pretend I didn't change my mind ten minutes later and move it a bit.)

There was a bit of messing about with colours, inevitably, but what I ended up with for the next part was two Gradient Map layers and a gradient.
The layer just above the base was a gradient, f6d0cd (pink) from the lower left, set to Multiply at 70%.

I then used 4c4455/f6e5cd (a greyish purple and a pale peach) for a Gradient Map, and duplicated this. Usually I go for a grey-blue, but having tried that, I wanted something a little dirtier here, hence the purple.

The lower one is set to Color Burn at 33%, the upper to Color at 100%. It actually took quite a while as there was a lot of swapping about before I settled on this! So that's the colouring complete.
As for the text, 'rebel' is in Bleeding Cowboys, 18pt, AV50 in black. Then I made some tiny text using Christopherhand at 3pt, AV200, then copied this layer and flipped it horizontally to get more text.

Save for Web and Devices as .png24, and there we are.
